The PB4 is a document that establishes an agreement between Brazil and Portugal, Cape Verde, and Italy. Although commonly known as PB4 or PT-Br/13, its official name is CDAM, which stands for the “Certificate of Right to Medical Assistance”. It ensures that Brazilian citizens can use the public health systems in these countries under the same conditions as local residents. This document is highly recommended for anyone planning to visit or live in one of these countries, offering coverage for medical consultations and emergencies.

The PB4 acts as an extension of Brazil’s public health system (SUS) in countries like Portugal and Italy. It guarantees that Brazilians can access public medical services in these nations without the high costs typically charged to foreigners.
In Portugal, the PB4 allows Brazilians to access the National Health Service (SNS). This includes medical consultations and care at public hospitals. For minors, they must be included as dependents on the PB4 of their parents or guardians, and the child’s birth certificate must be presented.
The application is made through the Brazilian Federal Government’s official website, and the form is easy to complete. After submission, the review and approval may take up to 30 calendar days.
If you don’t live in Brazil and need to prove your address, you can use a document in the name of a parent or spouse. Then, attach documents proving the relationship, such as a birth or marriage certificate, in the “Additional Information” field. If you don’t have proof of address in your name or a family member’s, download and fill out the residence declaration available on the government website.
The PB4 is free and, once requested, is usually ready within 30 calendar days. Its validity is one year (365 days) but can range between 6 months and 5 years, depending on the Federal Government’s instructions at the time of issuance. Renewal can be done directly through the website, following the same steps as the initial application.
The PB4/CDAM/IB2 is an excellent benefit for those planning to live in Portugal or Italy, but it does not replace travel insurance. While the PB4 provides public healthcare coverage, travel insurance typically includes broader services, such as medical repatriation, baggage loss coverage, and legal assistance.
